Researchers at the Medical College of Wisconsin will be conducting research determining the safety and feasibility of hypotensive fluid resuscitation compared to standard fluid resuscitation in patients with traumatic shock starting April 2012 in Milwaukee County. Patients with blunt or penetrating injuries and a systolic blood pressure ≤90 will be enrolled. In the hypotensive resuscitation group, patients will receive 250mL of normal saline rapidly if their radial pulse is absent or their systolic blood pressure falls below 70mmHg. This process using small bags of fluid will be repeated until a radial pulse is present or until 2 hours after hospital arrival or hemorrhage control has been achieved. In the standard resuscitation group, patients will receive 1000mL of normal saline to maintain a systolic blood pressure of 110mmHg. This process will be continued for 2 hours after hospital arrival or until hemorrhage control is achieved.
This study meets the FDA guidelines for exception to informed consent under emergency circumstances because informed consent cannot be obtained from a victim of sudden cardiac arrest.
